Please fill out the form below prior to attending the birthday party. All participants must have submitted a completed form prior to taking part in one of Craven Gymnastics Club Parties.

By submitting this form, you acknowledge and agree to the terms of the activity waiver and release.

Please wear correct attire and bring a water bottle if possible.

I understand that participation in a gymnastic party involves inherent risks, and I voluntarily consent to my child's participation. I release the organisers from any liability for injury or loss arising from participation in the activities.

I confirm that I have read and agree to the Medical Attention Waiver.

I hereby give my permission to Craven Gymnastic Club to procure medical attention for my child in the event of an injury, accident, and/or illness to my child while participating in Craven Gymnastic Club activities which, in the opinion of a paramedic or a medically trained practitioner, requires urgent medical attention and a parent, legal guardian or any given emergency contact are not available.
